Comprehensive surgical management in mixed ocular burn with severe symblepharon

Abstract

Objective. The aim of this article is to present a clinical case of severe ocular burn managed with early comprehensive surgical intervention, highlighting its evolution and correlation with current literature.

Case report. A 33-year-old male presented with a mixed ocular burn caused by boiling water and coolant, showing total corneal opacity, severe symblepharon, and 180° inferior limbal ischemia. Comprehensive surgical management was performed in a single procedure, including conjunctival debridement, amniotic membrane transplantation, and placement of an Illig conformer. Follow-up revealed progressive limbal revascularization, adequate epithelialization, anatomical preservation, and visual recovery, reaching uncorrected visual acuity of 7/10 at 21 days.

Conclusion. In this patient, early surgical intervention with conjunctival debridement, amniotic membrane transplantation, and an Illig conformer successfully controlled acute inflammation, prevented symblepharon, and enabled useful visual recovery within three weeks, preserving ocular surface integrity.
