Efficacy of 360° laser retinopexy in rhegmatogenous retinal detachment

Abstract

Objective: To evaluate the efficacy of 360° laser retinopexy in primary vitrectomy in patients with rhegmatogenous retinal detachment (RD) without explant.

Methods: A retrospective, non-randomized, case series study of patients operated for rhegmatogenous RD by 23G pars plana vitrectomy, with 360° laser retinopexy, was performed between January 2018 to December 2020, who had 12 months of postoperative follow-up. The main parameters to be evaluated were re-detachment recall, retinal status, macula (adherent or not), and best-corrected visual acuity (BCVA).

Results: A total of 36 eyes were included (36 patients, 22 phakic and 14 pseudophakic), with a mean age of 58.5 ± 13.19 years (28-83). In 3 eyes (8.3%), re-detachments were detected (1 at 1 month and 2 at 3 months after surgery), which were reoperated. At one year the retina was attached in all cases (with attached macula). In 29 eyes the preoperative BCVA was ≤ 0.3 decimals, (in 21 cases less than 0.1 decimals). After 1 year of surgery, 8 eyes had a best-corrected VA ≤ 0.3 decimals (only 1 case less than 0.1 decimals), with 9 eyes obtaining between 0.4 to 0.6 decimals and 19 eyes ≥ 0.7 decimals.

Conclusions: Retinopexy 360° associated with 23G vitrectomy was effective for retinal application, without recurrence, 1 year after surgery, obtaining a significant visual gain, in the series presented, with a re-detachment rate of 8.3% in the first trimester.
